What is the Braden Score?

The Braden Score is a widely used assessment tool in healthcare to predict a patient’s risk of developing pressure ulcers, also known as bedsores or pressure sores. Developed by Barbara Braden and Nancy Bergstrom in 1987, this scale evaluates six key factors that contribute to pressure ulcer formation: sensory perception, skin moisture, activity, mobility, nutrition, and friction/shear. By assigning scores to each of these factors and summing them up, healthcare professionals can quickly assess a patient’s risk level and implement appropriate preventive measures.

How to Use the Braden Score Calculator

Step 1: Assess Sensory Perception

Evaluate the patient’s ability to respond meaningfully to pressure-related discomfort. Choose the most appropriate option from the dropdown menu, ranging from “Completely Limited” to “No Impairment.”

Step 2: Evaluate Skin Moisture

Determine the degree to which the patient’s skin is exposed to moisture. Select the option that best describes the patient’s condition, from “Constantly Moist” to “Rarely Moist.”

Step 3: Gauge Activity Level

Assess the patient’s degree of physical activity. Choose the most fitting description, ranging from “Bedfast” to “Walks Frequently.”

Step 4: Determine Mobility

Evaluate the patient’s ability to change and control their body position. Select the appropriate option, from “Completely Immobile” to “No Limitation.”

Step 5: Assess Nutritional Status

Consider the patient’s usual food intake pattern. Choose the option that best reflects their nutritional status, from “Very Poor” to “Excellent.”

Step 6: Evaluate Friction and Shear

Assess the patient’s risk of friction and shear forces. Select the most appropriate option, from “Problem” to “No Apparent Problem.”

Step 7: Calculate and Interpret Results

After selecting options for all six categories, click the “Calculate Braden Score” button. The tool will display the total Braden Score and the corresponding risk level. Review the interpretation provided, which explains the risk categories and their implications.

Step 8: Use Results for Care Planning

Based on the calculated risk level, work with the healthcare team to develop an appropriate pressure ulcer prevention plan. This may include more frequent repositioning, specialized support surfaces, or enhanced skin care protocols for high-risk patients.
