RVU System Overview

The Relative Value Unit (RVU) system is the foundation of the Medicare Physician Fee Schedule and many private payer reimbursement models. It provides a standardized method for determining physician compensation based on the relative resources required to provide medical services.

Components of RVU

Each CPT code is assigned three distinct RVU components:

  • Work RVU: Reflects the physician’s time, effort, skill, and stress involved in providing the service
  • Practice Expense RVU: Covers overhead costs including equipment, supplies, and non-physician staff
  • Malpractice RVU: Accounts for professional liability insurance costs

Geographic Practice Cost Indices (GPCI)

GPCI adjustments account for geographic variations in costs across different locations. Each RVU component is multiplied by its corresponding GPCI value:

  • Work GPCI: Adjusts for geographic differences in physician work costs
  • Practice Expense GPCI: Adjusts for local practice expense variations
  • Malpractice GPCI: Adjusts for regional malpractice insurance cost differences

Payment Calculation Formula

The Medicare payment amount is calculated using the following formula:

Payment = [(Work RVU × Work GPCI) + (PE RVU × PE GPCI) + (MP RVU × MP GPCI)] × Conversion Factor

Facility vs Non-Facility Settings

Practice Expense RVUs differ between facility and non-facility settings:

  • Facility: Services provided in hospitals, ambulatory surgical centers, or other facilities where the facility bears most overhead costs
  • Non-Facility: Services provided in physician offices or other settings where the physician bears the overhead costs

Current Conversion Factor

The Medicare conversion factor is updated annually and converts the total RVUs into dollar amounts. The 2024 Medicare conversion factor is approximately $33.89.
